Approval/Reimbursement Form

In order for AAPA to distribute applicable regional escrow funds, we need to receive the approval/reimbursement form.
Please allow six weeks for approval and processing of request.

Flat Grant Information

A flat grant can be spent at the discretion of the constituent organization receiving the grant. A flat grant does not require itemization or receipts.

1. Please explain what the grant will cover.


Travel assistance for constituent organization representative to attend which activity?
Who is the person and what is their position?
What is the destination?
What dates are covered?
What is the dollar amount of the grant?

Itemized Grant Information

To be considered, all submitted itemized grant fund requests need to be have supporting receipts. Please mail reciepts separately to Sue Curtis, AAPA, 950 North Washington Street, Alexandria, VA 22314-1552.

2. Please specify what expenses the itemized grant covers.
(e.g., hotel deposit, printing, brochures, postage, speaker).

3.To receive an itemized reimbursement grant, the recipient must submit the following information and receipts.

1. What method was used to decide how the funds would be spent?

2. List who participated in the approval process and their role
(e.g., CRC representative, regional planning committee member, chapter representative).
