Constituent Organization Awards of Achievement General Criteria

Constituent Organization Awards of Achievement (COAA) seek to recognize AAPA constituent organizations that have developed and successfully executed innovative programs or projects with demonstrated overall positive results in health care, the PA profession, volunteer leadership development, and constituent organizational growth.

COAA application process contains three steps and each must be completed in its entirety:

     1.  Download and complete the COAA Activity Plan.
     2.  Complete the COAA Application form below.
     3.  Upload a completed COAA Activity Plan and supporting documentation in COAA Application.
     4.  Submit.

You will receive an automatic confirmation email. If you do not receive confirmation notice, please contact Linn Wheeling at lwheeling@aapa.org or call 571-319-4432.

You have the option of completing your entry form in sections by creating an account, which allows you to log in and out during multiple work sessions. You may save your progress by selecting 'save partial work' at the end of each page. Please note that your entry form will not be electronically submitted until you click 'submit' on the last page of the entry form.

Deadline for all COAA submission forms is 12:00 AM Eastern Daylight Time Zone March 16, 2012. 
 


All award applicants will be contacted regarding the status of their submission on or before April 15, 2012.
