IMPORTANT NOTE ON FILLING OUT THIS FORM: In an attempt to streamline the judging process, we are collecting everything online here.
You need to complete the form in one sitting, so don't start until you have read the next paragraph.
INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED.
This form includes several detailed questions and the optional uploading of files - as well as the submission of a supporting note from an editor of a U.S. or Canadian media outlet for those of you who are full-time employees and a letter of interest from an editor of a U.S. or Canadian media outlet if you are a freelancer. We suggest you read through the questions first, then compose the detailed answers using a word processor (or, where applicable, Excel) and then come back here to fill in the form. If you are a full-time employee, you will need to cut-n-paste or attach a document from your editor, so you should collect that first.
