FormSite.com is an easy-to-use online tool for building and hosting online forms and surveys. Our system enables all types of users to create a form and capture the results. There are three main parts of FormSite.com:

Create Your Form

Above is a picture of our New Form screen.

1. Give your form a name. This is an internal name that you will use to identify the form within FormSite. Give this form a unique name, such as "Invoice 2007," or "Customer Feedback Survey."
2. Choose the base for your form. Select from our list of templates, copy an existing form, or start from scratch.
3. Browse the templates. Click the name of a form to select your template, or click on "Preview" to see what the form looks like.
