2008-2009 Coach Registration
This form is to be used to register all coaches online. You must have a 2008 Team Identification Number (TIN) to complete this form. In order to obtain a TIN, the coach/team manager must register your team online. All coaches must also submit required CYSA 1628 form *rev. 12/2007, copy of their coaching license, and a 1x1.5" photo of passport quality at the team registration appointment.
