2009/10 Coach Sign-up

This form is to be used to sign-up all coaches online. You must have a 2009 Team Identification Number (TIN) to complete this form. In order to obtain a TIN, the coach/team manager must register your team online. To register, coaches must also submit a signed Coach Registration/Team Management Disclosure Form 1628 (and a copy), a passport size/quality photo, and a copy of their coaching license to the team manager in time for the team registration appointment. These items are necessary for Vikings to create the necessary "coach pass". Managers/Primary Team Contacts not planning to do any coaching only need to submit the signed 1628 form.
