2010/11 Player Sign-up
You must have a 2010 team name and Team Identification Number (TIN) to complete this form. In order to obtain a TIN, the team's coach/team manager must register the team online. If you do not have a team, do NOT use this form. In order to register, every player on a team must complete this form online and submit passport size/quality photo, check for appropriate fees, 2 registration forms #1601 signed in INK, copy of birth certificate or passport, and signed Code of Conduct (for fall play only) to the Coach/Manager in a timely manner.
