2010/2010 Vikings CLUB Team Registration

This form is to be used by a manager/coach to register his/her team if the team is a Club Travel and CLUB upper house teams playing in a league usually other than Vikings; the exception being Upper House level of play. If you are registering a recreational team that will be playing in the Vikings League, go back on your browser to find the correct instructions. Within three business days of completing this form, the Manager/Primary Team Contact will receive a Fall 2010 Team Identification Number (TIN) via e-mail.
