Hit the ground running with an example template. Start with one of these forms to see a feature in action and customize it to create a powerful form quickly.
All Item Demo
We offer a lot of customizable items and powerful features. Our All Item Demo template is a great way to get started. It features an example of nearly every item type so you can see them in action. Even if you don’t intend to use every item on the form, you can use this as a jumping off point by customizing it to fit your needs. Sometimes it’s helpful to experiment with settings, or just to quickly get a glimpse of some of what is possible. For more details see our Support and Features pages.
File Upload Example
Accept files from your customers' and users' form submissions with a File Upload item. Easily manage application submissions or registration campaigns. With our mobile responsive forms, you can even have your users submit photos directly from their phones. This is a great way to handle field inspections and reports. With our ability to embed your form on a personal site or post to Facebook and Twitter, you can even use this to collect contest submissions or social media feedback from your followers from any device. With a File Upload item you can accept any type of file or restrict it to a certain type of file or image. You can also allow your users to upload multiple files at once or limit how big their files are depending on your needs. Once you collect a file you can have it emailed to you or send it to a file storage service like Dropbox, Box, Google Drive, or OneDrive. This can be used to store a backup or to share with others using your existing accounts. For more details see our Support and Features pages. See our File Integration Support pages for more help sending your files to other services.
Rating / Ranking Example
Easily conduct questionnaires and evaluations using a Rating or Ranking item. With a multitude of item types, you can create customized and in-depth forms and surveys to gather valuable data and feedback. Ratings allow you to prompt your users to assign points, or weightings, to choices. This can be used to allocate portions of a resource to different options or categories. Rankings let users order a list of choices. This can be a great way to let your users vote or assign favorites. For more details see our Support and Features pages.
Multi-scale Example
Collect detailed information about groups of topics with a Multi-Scale item. You can create a form that collects complex input easily. Multi-Scale items allow you to enter multiple sections in a single item so that you can capture multiple data points but keep them grouped together. This can help you collect detailed information while keeping it easy both for you and people filling out your forms and surveys. For more details see our Support and Features pages.
Notifications Example
Be notified when your forms and surveys are submitted by receiving an email. You can have a Notification sent to yourself, back to the person who filled out the form, or a third party. This is a great way to know when a new order comes in or someone signs up for your program. You can also send more details to the user as a receipt or include special instructions. With a Notification, you can customize the information that will be sent in the email. Choose from one of several formats to show the results in or simply add a custom message. If you accept files on the form you can have them attached to the email. You can also add Conditional Logic Rules so that a given email will only be sent out when the form was filled out a certain way. In addition to this, we also offer special Email Routing items that can have the emails sent to a different person based on what options the user selects on the form. All of this combined allows you to setup extremely simple or complex email campaigns that will all happen automatically as your form gets filled out. For more details see our Support and Features pages.
Piping Example
Piping is a simple feature that allows you to do powerful things. At its simplest, Piping allows you to take what the user entered in one item and insert it into another item. You can use this in a lot of interesting ways. Set the default value for an item to what the user typed in earlier, or show the user’s name or other details in a section of text or an item’s question. This can provide user-friendliness or a better level of personalization while the user fills out the form. You can also pipe in the user’s responses into custom Notification email or Success Pages messages, or even into redirect URLs that can be used to send data to another site. In addition, we also have “smart” Piping features such as the ability to conditionally set a default in case the user doesn’t enter any info into the source item. You can even pipe extra information from the result such as its unique Reference # value, order total or score, among other data. For more details see our Support page.
Funneling Example
Enhance your forms with advanced features. Funneling is perfect for surveys or questionnaires, or anytime you want to collect in-depth details. Your form will adapt to the user with no custom coding required. Funneling can be used to drill down, or ask follow-up questions, based on the user’s responses. You can add a multiple choice item like a Checkbox or Radio Button, and then later on the form prompt for more input for only the choices that the user selected. Usually this would take a lot of effort to setup, but you can do this easily with our Funneling feature. Just add the items, link them with the Funneling option, and you’re done! For more details see our Support page.
Calculations Example
Make powerful forms by processing equations based upon your user’s input. With the Calculations feature, you will be able to produce output dynamically based on how users fill out your form. Calculate the material needed to cover a customer’s order, perform measurement conversions, or calculate a custom equation automatically. The Calculation item allows you to enter an equation and you can add input from fields the user filled out along with constants that you define. You can even add multiple Calculation items with different equations or that use the output from one as input into the next. Save the Calculation output with your results, or optionally add them into the Order or Scoring total to enhance those features even further. For more details see our Support and Features pages.
Item Rules Example
Make dynamic forms using Rules. Item Rules can be used to conditionally show or hide items on the form based on responses to previous items. Smartly handle optional fields or branching logic by hiding items and then only showing them when needed. You can also make the form look customized to the user and how they are filling it out as they proceed. This is a great way to encourage engagement on your forms and help to collect responses. Item Rules are also a simple way to collect follow-up information across multiple items. For example, if you have an order form selling multiple items, you may only need to collect specific options for certain items. Instead of showing every option all the time, use an Item Rule to show only the options that are relevant for the items the user is purchasing. For more details see our Support page.
Page Rules Example
Make dynamic forms using Rules. Page Rules can be used to send the user through different pages of a form based on their response. Smartly handle optional fields or branching logic by grouping items onto separate pages and then use Page Rules to send the user only to the pages relevant to them. By skipping over unnecessary pages, users will be able to fill out the form customized to them. Repeating Page Rules can also be used to repeat a page based on the user’s entries. Rather than duplicating one or more items, you can add fields to a page, and then set the entire page to repeat as much as needed. This is a great way to handle registrations for families or groups. If you need to collect additional information for each member, you can easily do this in a Repeating Page and let us take care of the work. For more details see our Support page.
Same Page Rules Example
Create complex forms simply. Use our Rules features to set up forms to respond to the user’s inputs. This can be a huge help for almost any form or survey where you want to ask for targeted information. With the Rules logic you can still have a detailed form while keeping it easy for your users to fill out. By adding Rules to items on the same page, you can have items show or hide based on how the user fills out the form. This can be used to control the user’s path through the form, or present a customized list of questions. For more details see our Support page.
Repeating Pages Example
With a Repeating Page you will be able to collect detailed information easily. Set up a group of items to repeat based on how the user fills out the form. This can let you easily handle in-depth registrations and signups for multiple people, or collect information on multiple items, all in a single form. Normally this would mean you need to duplicate a lot of fields on a form, but with our Repeating Pages feature you can just set up the items once then add Rules to repeat the page as needed based on the user’s entries in the form. Once enough repetitions are collected, the user can continue through the form normally or you can send them to a new page with another Rule. You can even configure multiple pages to repeat. When you are viewing results, you will be able to see each grouping separately so that you can easily manage your data. For more details see our Support page.
