We offer a lot of customizable items and powerful features. Our All Item Demo template is a great way to get started. It features an example of nearly every item type so you can see them in action. Even if you don’t intend to use every item on the form, you can use this as a jumping off point by customizing it to fit your needs. File Upload Example
Accept files from your customers' and users' form submissions with a File Upload item. Easily manage application submissions or registration campaigns. With our mobile responsive forms, you can even have your users submit photos directly from their phones. This is a great way to handle field inspections and reports. With our ability to embed your form on a personal site or post to Facebook and Twitter, you can even use this to collect contest submissions or social media feedback from your followers from any device. With a File Upload item you can accept any type of file or restrict it to a certain type of file or image. You can also allow your users to upload multiple files at once or limit how big their files are depending on your needs. Once you collect a file you can have it emailed to you or send it to a file storage service like Dropbox, Box, Google Drive, or OneDrive. Rating / Ranking Example
Easily conduct questionnaires and evaluations using a Rating or Ranking item. With a multitude of item types, you can create customized and in-depth forms and surveys to gather valuable data and feedback. Ratings allow you to prompt your users to assign points, or weightings, to choices. This can be used to allocate portions of a resource to different options or categories. Multi-scale Example
Collect detailed information about groups of topics with a Multi-Scale item. You can create a form that collects complex input easily. Multi-Scale items allow you to enter multiple sections in a single item so that you can capture multiple data points but keep them grouped together. Admin Items
Collect extra data with your form results that users can’t see. Sometimes your colleagues may need to make notes on a particular submission, or you need to add in extra information for an order but you don’t want your customer to see it. By setting an item as an Admin Item, you can add protected fields to your form that only you or admin users can see. This is a great way to handle complex processes or just an easy way to add notes about each respondent. Admin Items will be completely hidden from your users when they are filling out the form. Only when you view results formats will they be shown, and you can add information when editing a result.
